The Long Term Conditions clinic usually takes place each afternoon, between 1.30pm and 4pm. If you have a long term condition (asthma or other respiratory condition, diabetes, heart disease or have had a stroke or mini-stroke) you will be invited once a year for an annual check with a specialist nurse. The check takes approximately 20 minutes.
A blood test is required 2 weeks before the clinic appointment for patients with heart disease, diabetes or having had a stroke. You will be advised of this when you ring to book for the clinic. Please note you will be given the results of this blood test in the clinic, there is no need to telephone for them.
Please be aware that we are required by the government to call you three times each year if you do not respond to our letters. If you do not want to be called or you have had a letter and don't want to come to clinic you can download a disclaimer form from here. Fill it in and return it to us and we won't call you again for a year.
